At Matilsky, we’re happy to serve our patients and do everything we can to make your office visit a smooth and pleasant experience. In order to provide you with professional and efficient service, please review the policies and frequently asked questions below.
How do I schedule an appointment?
Appointments can be made by phone or in person. For your convenience, we will soon be offering appointment scheduling on our website. Walk-ins will be seen in order of arrival once all appointments have been met. Upon arrival, please inform the medical assistant of the reason for your visit.
What if i need to change or cancel my appointment?
If for any reason you must cancel or reschedule your appointment, please call us at least 24 hours prior to your scheduled appointment. A fee may be charged if you do not cancel within 24 hours prior to your scheduled time.
What if I am first-time patient?
We will send you the new patient packet by email, mail or fax. Please complete it and bring it with you to your first visit. If you did not receive your packet, please arrive 30 minutes before your scheduled appointment. Please bring the following items to your first visit:
- insurance card
- driver's license or photo identification
- medical history
- all prescriptions medications that you take in their original bottles
- a list of all over the counter medications you take
- medical records from previous doctors, if available
- a list of other doctors you're regularly seeing
What if I am a returning patient?
Please bring the following items to your visit:
- insurance card, if changed
- all prescriptions medications that you take in their original bottles
- a list of all over the counter medications you take
- medical records from other doctors that you've seen since the previous visit
Returning snowbirds, please remember to bring medical records from the time you've been away.
What do I need to prepare for my yearly physical exam?
When you schedule your early exam you must also schedule your yearly blood work. Our office will provide you with the necessary requisition forms.
How much time dos it take to get my medical records from another doctor?
It can take up to one month for another doctor to provide us with your records.
How can I refill my prescriptions?
Prescriptions are refilled at office visits.
Can I call and talk to the doctor about my condition, instead of coming in?
We practice personalized care, which can only be provided at office visits and not over the telephone.
When is the office closed?
Our office is closed on Saturdays, Sundays and all national holidays.
What if I have an emergency?
Our phone is answered 24/7 for emergencies, but if you are having a life-threatening emergency you should dial 911.
